I have an ethnic middle eastern nose and I am looking to plan my 4th rhinoplasty.

I have had 3 rhinoplasties in the last 3 years. I am looking to plan my final one in a few months to address an issue I have.

I have an ethnic middle eastern nose. I find the base of the nose to be a bit wide. I also have no tip definition. I would not want to feminise my nose, however I believe I could add definition and narrow the base of the nose a bit with good results.

What sort of procedure does this involve? Could you briefly explain to me how this is done. Would you suture the tip cartilages together to achieve a pointier tip?

Since you are looking at the 4th rhinoplasty intervention, there is a possibility that the cartilage support inside your nose has become insufficient to create definition. Suturing the tip cartilages together only works as long as there is enough support to hold against the pressure of the skin, in particular if the skin (soft tissue envelope of the nasal tip) is thicker.

In a situation like this, I would favour using rib cartilage graft to provide support.

For narrowing the alar base reduction of the nostrils is the technique used.
