Question

I had primary rhinoplasty in 2009 to reduce a nasal hump and refine the tip with open rhinoplasty.

I have never been entirely happy with the results. I feel that my columella hangs now with too much nostril show, I also feel that the tip projects too far from my face.

I have been told by my previous surgeon that there was no more he could do but I would appreciate a second opinion. This is something that bugs me every day and I would just like to know if I just have to live with it or whether something can be done. I would also consider chin implant if it would help with the tip projection.

Answer

I would be happy to see you in consultation and evaluate using computer imaging and simulation what it is that you feel would represent a good end result and based on that to discuss feasibility and technical aspects related to a possible intervention.

There is no doubt that between computer simulations and the surgical outcome there are always differences, but clarity regarding the design endpoint provides a more structured basis for the decision regarding further surgery.

With regards to the chin augmentation, I tend to carry out the imaging in 3-D, and this can also be done to integrate the potential reshaping aspects for the nose.
